What days of the week are best for your musical growth submission videos? You will be assigned one of them, which will remain the same for the whole investment package. (You can turn it in early, but late results in a skipped week).

4 submissions and feedbacks (must be once per week - no makeup weeks) ($125 one time fee)
12 submissions and feedbacks (must be once per week - no makeup weeks) ($315 one time fee)

Once we receive this form, a letter of denial or acceptance is sent (we want to help you best). A link for payment is sent. We start on the stated date if the invoice is paid. Do you agree to this?

An instructor is not required to teach any particular piece if they do not see it as a great educational fit for you or have any moral or other conflict with it or it's message. Do you understand and agree to this statement?
